CareOregon Partners with GSI Health to Implement Critical Care Coordination Strategy
Philadelphia, PA – November 27, 2018 — GSI Health, a leading population health management technology company, today announced that its GSIHealthCoordinator platform has been implemented to manage care for CareOregon, a nonprofit organization supporting almost a third of Oregon’s Medicaid population. As the largest provider of Oregon Health Plan services, CareOregon supports four coordinated care organizations (CCOs) serving nearly 300,000 Medicare and Medicaid members across the state.
After a national search, CareOregon selected GSI Health as a partner to address the criticality of care coordination in its population health management strategy. The GSIHealthCoordinator platform integrates key systems and people to improve efficiency, breaking down siloes and enabling collaboration among disparate teams spanning the health plan’s business lines and community-based partners. GSIHealthCoordinator helps CareOregon put analysis into action, directing how care teams work together with harmonized workflows to proactively address all issues, including lifestyle and socioeconomic factors that can affect member health.
“We had built a team with the right players, and were looking for the right platform and experts to realize our vision,” said Amit Shah, Chief Medical Officer of CareOregon. “GSI Health brings a deep knowledge of population health management that we can leverage as we forge our way in the continually evolving world of value-based care.”
“GSI Health knows care management—they’re the right thought partner as we move beyond analytics to take our value-based care program to the next level,” said Jonathan Weedman, Director of Clinical Operations, Population Health Partnerships for CareOregon. “The platform enables us to fundamentally change how we deliver care management through integrated care teams. GSI Health distills data from multiple sources and lines of business, giving our teams the platform to more effectively work together for the health of our members.”
GSI Health’s GSIHealthCoordinator platform enables health plans to effectively manage members with complex issues by coordinating their care and identifying and addressing the medical, behavioral, and social determinants that impact the cost of care. The platform integrates disparate systems to provide transparency across the entire team regardless of the setting, supporting real-time information sharing and insights from across the community, including clinical data to improve care planning. Robust analytics and risk stratification enable identification of at-risk members for timely and appropriate interventions to reduce unnecessary admissions.
“This partnership reinforces the tenet that care coordination at the center of population health management is critical for managed-care organizations as well as providers,” said LeRoy E. Jones, Founder and CEO of GSI Health. “CareOregon’s focus on proactive care coordination for rising-risk populations using GSI Health as an enabling technology will make a positive impact on the health of its members, and we are excited to be a part of it.”
